A captain’s choice to obtain underway while exhausted brought about the grounding and also sinking of an angling vessel near Whittier, Alaska, the National Transportation Safety Board stated in Marine Investigation Report 22/02 launchedTuesday On July 24, 2021, the angling vessel Tenacious, which fished largely for pink salmon, based at the entry to Wells Passage while transiting to angling premises in Prince William Sound.

The 5 team participants deserted the vessel and also were saved. The Tenacious later on sank. One small injury was reported. Loss of the vessel and also angling equipment completed $660,000 and also the 2,000 gallons of gasoline onboard were not recuperated.

The Tenacious team showed up in Whittier late on July 22 to fix upkeep concerns with their skiff in advance of the opening of the fishery on July 24. Three of the team made a 4-hour big salami to Seward, Alaska to obtain one more skiff. According to a deckhand, the entire team was running “not too much sleep.” About 2 hrs after obtaining underway for the July 24 fishery, the Tenacious struck rocks. The captain informed private investigators he went to sleep. The vessel started to tackle water and also initiatives to quit the flooding were not successful. The team boarded the skiff and also were saved by the angling vessel Wildlife.

The NTSB established the possible root cause of the grounding was the captain’s choice to obtain underway while tired out. Contributing was the choice not to utilize the navigating system’s cross-track mistake alarm system and also to run with a non-functioning bridge watch alarm system.

“Even though captains face economic pressures, operating vessels while fatigued can be dangerous,” stated Morgan Turrell, supervisor of NTSB’s Office ofMarine Safety “The crew of Tenacious was fortunate no one was injured; others have not been so fortunate. Owners/operators should ensure crewmembers receive enough rest to adequately perform duties.”

There were 2 devices that might have stopped the grounding:

  1. Tenacious was equipped with a graph plotter, which was not being used, that consisted of a cross-track mistake alarm system. It would certainly have aesthetically and also audibly signaled the wheelhouse driver when the vessel left an established range off its organized track; and also
  2. The bridge watch alarm system, likewise not being used, establishes periods to guarantee the driver has actually not dropped off to sleep. This might have signaled the captain in time to potentially avoid the grounding.

Additionally, the Tenacious was not needed by policies to have water tight bulkheads; nonetheless, had the bulkhead in between the bunkroom and also the engine space been water tight, it would certainly have included the flooding.

Improving angling vessel safety and security stays a top priority for NTSB and also it is a problem on the NTSB’s 2021-2022 Most Wanted List ofTransportation Safety Improvements NTSB supporters for brand-new criteria to attend to– and also regularly reassess– undamaged security, neighborhood, and also water tight stability in industrial angling vessels as much as 79 feet long.
